• Good Faith Out-of-Pocket Estimate with insurance details and any insurance benefit information that is your responsibility.

• If you selected a therapist, you will receive an email with more details about your appointment details.

• By Federal Law, we are required to obtain certain document. You are invited to complete three forms: Consent to Treatment, Confidential Communications, and our General Intake Form.

Please complete these forms prior to attending your session. This will only take 3 minutes and you cannot be seen without these.
